Tekrarlayan İnmenin Önlenmesinde Kronik Bakım Modeli: Özyönetim Destekli Bakım Yaklaşımları

Abstract



Kronik Bakım Modeli, inme gibi kronik hastalığı olan bireylerin bakım süreçlerini ve sonuçlarını iyileştirmeyi amaçlayan çok bileşenli girişimler bakımından etkili bir çerçevedir. Modelin inme sonrası hasta sonuçlarını geliştirmeye adapte edilebileceği düşünülmekle birlikte, modele ait “özyönetim desteği” bileşeni kapsamında gerçekleştirilen girişimlerin, tekrarlayan inmenin önlenmesine katkı sağlayacağı söylenebilir.
