DİZ OSTEOARTRİTİ OLAN HASTALARIN AĞRI İLE İLİŞKİLİ AKTİVİTE PATERNLERİNE GÖRE SEMPTOMLARI, AĞRI ŞİDDETİ, HAREKET KORKUSU, ANKSİYETE, DEPRESYON DURUMU VE FİZİKSEL AKTİVİTELERİNİN KARŞILAŞTIRILMASI

Abstract

Amaç
Diz osteoartirti (OA) olan hastalarda ağrı ile ilişkili aktivite
paternlerinin incelenmesi bu hasta popülasyonunda
uygun rehabilitasyon programlarının oluşturulmasına
yardımcı olacaktır. Bu çalışmanın amacı diz
OA'lı hastalarda ağrı ile ilişkili aktivite paternlerine göre
semptomları, ağrı şiddeti, hareket korkusu, anksiyete,
depresyon durumu ve fiziksel aktivitelerinin karşılaştırılmasıdır.
Gereç ve Yöntem
Bu kesitsel çalışmamıza klinik ve radyolojik olarak diz
OA olan 120 katılımcı alındı. Diz OA olan katılımcılar,
Ağrı-Aktivite Paternleri Ölçeği’ne göre kaçınma paterni,
aşırıya kaçma paterni, düzene sokma paterni olmak
üzere üç gruba ayrıldı. Diz OA olan katılımcıların
ağrı şiddetleri Görsel Analog Skala, diz fonksiyonları
ve diz ile ilişkili yaşam kalitesi KOOS (Diz İncinme ve
Osteoartrit Sonuç Skoru) ölçeği, fiziksel aktivite seviyesi
Uluslararası Fiziksel Aktivite Anketi Kısa Form
(UFAA-Kısa), düşme korkusu TAMPA kinezyofobi ölçeği,
anksiyete ve depresyon durumları Hastane Anksiyete
Depresyon Ölçeği (HAD) ile değerlendirildi.
Bulgular
Çalışmamızda tüm grupların ağrı süresi, ağrı şiddetleri
(dinlenme-aktivite), KOOS puanlarının benzer olduğu
görüldü (p>0.05). Gruplar arasında HAD-anksiyete ve
depresyon durumları, TAMPA puanları ve UFAA-Yürüme
puanlarının ise faklı olduğu görüldü (p<0.05). Aşırıya
kaçma paterni olan hastaların anksiyetelerinin diğer
gruplara göre daha yüksek (p<0.05), kaçınma paterni
olan hastaların depresyon durumlarının (p<0.05) ve
düşme korkularının daha yüksek (p<0.05) ve düzene
sokma paterni olan hastaların ise yürüme puanlarının
daha düşük olduğu bulundu (p<0.05).
Sonuç
Çalışmamıza göre düzene koyma grubunda depresyon
ve anksiyete düzeyinin daha düşük olduğu ve pozitif
psikososyal sonuçlarla ilişkilendirildiği görülmektedir.
Ancak düzene koyma davranışı sergileyen diz
OA'li hastalarda fiziksel aktivite seviyesinin en düşük
olduğu görülmektedir.

COMPARISON OF SYMPTOMS, PAIN INTENSITY, KINESIOPHOBIA, ANXIETY, DEPRESSION, AND PHYSICAL ACTIVITY IN PATIENTS WITH KNEE OSTEOARTHRITIS ACCORDING TO PAIN-RELATED ACTIVITY PATTERNS

Abstract

Objective
Examining pain-related activity patterns in patients
with knee osteoarthritis (OA) will help to establish
appropriate rehabilitation programs in this patient
population. The aim of this study is to compare
symptoms, pain intensity, fear of movement, anxiety,
depression status and physical activities according to
pain-related activity patterns in patients with knee OA.
Material and Method: 120 participants with clinical
and radiological knee OA were included in this crosssectional
study. Participants with knee OA were
divided into three groups according to the Pain-
Activity Patterns Scale: avoidance pattern, overdoing
pattern, and regulation pattern. Visual Analogue
Scale for assessing the severity of pain of participants
with knee OA, KOOS (Knee Injury and Osteoarthritis
Outcome Score) for assessing knee functions and
knee-related quality of life, International Physical
Activity Questionnaire Short Form (IPAQ-Short) for
assessing physical activity level, fear of falling the
TAMPA kinesiophobia scale and the Hospital Anxiety
Depression Scale (HAD) for anxiety and depression
were used.
Results
In our study, participants with knee OA were divided
into three groups according to pain, and all subgroups
of pain duration, pain severity (rest-activity), and
KOOS scores were found to be similar to each
other (p>0.05). HAD-anxiety and depression states,
TAMPA scores and IPAQ-Walking scores were found
to be different between the groups (p<0.05). It was
found that patients with exaggeration pattern had
higher anxiety than other groups (p<0.05), patients
with avoidance pattern had higher depression
(p<0.05) and fear of falling (p<0.05), and patients with
regulation pattern had lower walking scores (p<0.05).
Conclusion
According to our study, it is seen that the level of
depression and anxiety is lower in ordering behavior
and it is associated with positive psychosocial
outcomes. However, it is seen that the level of
physical activity is the lowest in patients with knee OA
who exhibit regulation behavior.
