YOĞUN BAKIM ÜNİTELERİNDE MEKANİK VENTİLATÖRE BAĞLI HASTALARDA VENTİLATÖR İLİŞKİLİ PNÖMONİ (VİP)’Yİ ÖNLEMEYE İLİŞKİN AĞIZ BAKIMI ETKİNLİĞİNİN İNCELENMESİ: SİSTEMATİK BİR DERLEME

Abstract

AMAÇ: Bu çalışmanın amacı, ventilatör ilişki pnömoniyi önlemede ağız bakımı etkinliğini değerlendiren mevcut literatürün gözden geçirilmesi ve çalışmalardan elde edilen verilerin sistematik biçimde incelenmesidir.
GEREÇ VE YÖNTEM: Çalışmanın evrenini, 2001-2017 tarihleri arasında “Google Scholar”, “Pubmed”, “Cochrane”, “Google Akademik” ulusal ve uluslararası veri tabanları taranarak ulaşılan makaleler oluşturmuştur. İncelemede “ventilatör ilişkili pnömoni (VİP), “ventilator associated pneumonia (VAP) prevention”, “ventilatör ilişkili pnömoni (VİP) ve ağız bakımı”, “ventilator associated pneumonia (VAP) prevention and oral care” anahtar kelimeleriyle VİP önleme protokol ve uygulamalarının sonuçlara etkisini değerlendiren, yayın dili Türkçe ve İngilizce olan ve tam metni bulunan makaleler seçilmiştir. Bu makalelerden araştırma kriterlerini karşılayan toplam 20 çalışma incelemenin örneklemini oluşturmuştur.
BULGULAR: Çalışmaya dâhil edilen makalelerde, randomize kontrollü, prospektif-randomize kontrollü, yarı deneysel, pilot-randomize kontrollü, çift kör-randomize plasebo kontrollü, çift kör-prospektif-randomize kontrollü, prospektif, çift kör- randomize kontrollü araştırma tasarımlarının kullanıldığı saptanmıştır. Bu sistematik derleme ile ventilatör ilişkili pnömoninin önlemesinde kullanılan klinik protokol ve koruyucu önlemlerden “klorheksidin”, “arıtılmış su”, “klorheksidinli swab”, “oral jel”, “sodyum bikarbonat”, “listerine”, “hidrojen peroksit”in ağız bakımında kullanılması etkili olduğu bulunmuştur.
SONUÇ: Bu sistematik derleme sonucunda elde edilen çalışmaların 9’unda kullanılan VİP’yi önlemede klinik protokol ve önleyici uygulamaların VİP sonuçlarına etki etmediği, 11’inde ise kullanılan klinik protokol ve önleyici uygulamaların VİP sonuçlarına olumlu yönde katkı sağladığı görülmüştür.
